Acknowledgment of Risk

I understand and acknowledge that participation in sports activities involves inherent risks, including but not limited to physical injury, accidents, or exposure to environmental elements. By completing this form, I voluntarily assume all risks associated with participation in these activities.

Release of Liability

I hereby release and hold harmless Athlete Apex Center LLC, its employees, agents, and volunteers from any and all liability for any injury, loss, or damage to my person or property incurred while participating in the activities.

Medical Authorization

In the event of injury or illness, I authorize Athlete Apex Center LLC to arrange for emergency medical treatment on behalf of the participant. I agree to bear all associated costs for such treatment.

Photo/Video Release

I grant Athlete Apex Center LLC permission to take photos and/or videos during the clinics/camps and use them for promotional or educational purposes, including social media, flyers, and the organization's website.
